PPO/EPO (Preferred Provider Organization)
Usually higher premiums but allow for maximum freedom in selecting a physician. Makes for easier referrals since most specialty physicians in this area are on most PPO/EPO plans.

HMO/POS (Health Maintenance Organization/Point of Service)
These programs are more restricted but more affordable. HMO/POS ask that you choose a physician within a network. That doctor, or primary care physician (PCP), is paid a monthly amount for signing with him/her. The finances are such that patients must stay within a network that the PCP is registered in and accept referrals to specialist that are within that network. Patients at times discover that even though a specialist may be in the insurance plan’s list of physicians it must be in the same network that the PCP is registered with for you to be able to see the specialist.

POS plans, slightly more expensive that HMO plans, allow you to go outside the network and see a specialist that will accept your insurance when you wish to do so and therefore acts like a PPO - you may need to pay a percentage of the cost for the visit as well as lab work and radiology so consider this expense as well. Most patients in POS plans get into a situation where an outside specialist may recommend an MRI and you may not wish to pay the extra cost and then ask your PCP to process this through the HMO. Most likely the PCP in this case will need to send you to a specialist within the network for a second opinion before he/she can get approval for an outside specialty request.

HMO Senior Plans (Health Maintenance Organization)
Like an HMO plan, seniors over 65 years of age or disabled patients on SSI, can sign over their Medicare health care benefits to a medical insurance company. Like an HMO, you must select a primary care physician within an HMO network and once selected payment for your services go from the US government to the health insurance company to the HMO network that you primary care physician is listed with.
